HEALTH REQUIREMENTS FOR STUDENTS ENTERING 6TH GRADE
For entry to 6th grade, your child must have:
1. Physical exam (Illinois Certificate of Child Examination)—must be completed within one year prior to the first day of school. This exam is good for 395 days for interscholastic sports participation. DUE: JULY 1st
YOUR STUDENT WILL BE EXCLUDED THE FIRST DAY OF SCHOOL IF THE PHYSICAL EXAM IS NOT ON FILE WITH THE NURSE.
2. Proof of the following immunizations—1 dose of Tdap, 1 dose of meningococcal vaccine, Hepatitis series of 3 doses completed, in addition to other required Illinois vaccines on the appropriate schedules.
Note: Students that have not yet turned 11 years old will need proof of an appointment to get the vaccines after the 11th birthday, BUT WILL REQUIRE THE PHYSICAL EXAM by the first day of school.
3. Dental exam submitted by May 15 of the school year. (Any dental exam done after November 15 of the prior school year is acceptable).
4. Vision exams are recommended but not required for sixth grade.
